People in Worcestershire can take control of their health using an online tool hosted by Diabetes UK to help calculate their risk of developing type 2 diabetes.
Diabetes UK’s Know Your Risk tool rates people according to their risk of developing type 2 diabetes. If your risk is moderate or high, you can refer to a local program that is part of the Healthier You: NHS Diabetes Prevention Program for easy access to free health and lifestyle coaching.
Everyone who takes part in the program has the choice between app-based one-on-one coaching and group sessions. Access to the program is completely free for users and available without consulting a doctor.
Across Worcestershire, this service is offered by Liva Healthcare, a personalized provider of digital health coaching, and personal educational support from the nonprofit health care provider Living Well Taking Control.
William Bachop, pre-diabetic and user of the app-based program, says: “My lifestyle has changed dramatically since I joined the program.
