This month, the North Country Chronic Disease Prevention Coalition will launch a new referral system to connect prediabetic and diabetic patients with local, evidence-based programs that can help them prevent or better manage their disease. The Heart Network thanks and appreciates the members of the coalition who helped develop this process by which providers can easily connect patients to these life-saving services.
A special partner in this effort, NY Connects, deserves special recognition for agreeing to manage this new referral program. NY Connects – a nationwide directory of health, home care, transportation, and other resources – will follow up on patients who have been referred to help them understand and enroll in community-based program options.
Preventing and treating diabetes is a top priority for the Chronic Disease Prevention Coalition. Not only do health care providers play a vital role in treating patients with prediabetes and diabetes, but they also help them find community-based services to help them get and stay healthy as they adopt daily routines and practices. Ultimately, we want these people to avoid their diabetes or at least get it under control themselves.
The coalition knows from existing research that many residents of the north have difficulty controlling their condition. As a collective, we believe this referral system will reduce these numbers and empower individuals to take control of their own health.
People with prediabetes or diabetes should also speak directly to their doctors through local services or visitkompasshp.org to find a program in their area.
The North Country Chronic Disease Prevention Coalition, supported by the Heart Network, brings together providers and other key players in the region to replicate and implement successful programs to treat patients with chronic diseases.
